Fertility After 40 — What Changes?
A woman is born with all the eggs she will ever have. As she ages, both the number and quality of eggs decline. This process accelerates significantly after age 35 and becomes more pronounced after 40. Key changes include:
- Reduced Ovarian Reserve: Fewer eggs remain available for stimulation, leading to lower egg numbers in IVF retrieval
- Declining Egg Quality: Eggs in older women are more likely to carry chromosomal errors (aneuploidy), which leads to lower fertilization rates, lower-quality embryos, reduced implantation, and higher miscarriage rates
- Lower AMH: Anti-Mullerian Hormone (AMH) levels fall sharply in the 40s, reflecting diminishing ovarian reserve
- Higher Miscarriage Risk: The risk of miscarriage rises from approximately 10–15% in women under 35 to 30–50% or more in women over 40, primarily due to chromosomally abnormal embryos
Despite these changes, many women over 40 do achieve successful pregnancies with appropriate fertility treatment at Pratham IVF Center.
